本研究旨在探讨加味七方胃痛颗粒阻断胃癌前病变(precancerous lesions gastric cancer, PLGC)“炎癌转化”的分子机制,明确其对胃癌前病变的调控作用。本研究通过复合造模法构建PLGC大鼠(Rattus norregicus)模型,随机分为对照组、模型组、维酶素组及中药组。采用HE染色法评估胃黏膜病理学改变,采用免疫组织化学技术检测p53、 Ki-67蛋白表达;通过转录组测序筛选差异表达基因(differentially expressed genes, DEGs),对DEGs进行功能注释和KEGG富集分析,并对富集到的关键信号通路进行验证。研究结果显示,与空白组大鼠相比,模型组大鼠体质量和3 h进食量均显著降低(P<0.01),胃黏膜腺体萎缩明显,Ki-67、 p53蛋白表达水平升高(P<0.01);与模型组相比,中药组和维酶素组的体质量和3 h进食量显著增加(P<0.01),胃黏膜腺体萎缩改善,炎性细胞浸润数量减少,Ki-67、 p53蛋白表达水平降低(P<0.01)。转录组学测序显示,和模型组相比,空白组中发现了1 197个DEGs;和模型组相比,中药组中有1 298个DEGs。对两组DEGs进行多基因富集可视化,发现PI3K-Akt信号通路显著富集。Western blot结果显示,与正常组大鼠胃组织相比,模型组大鼠胃组织p-PI3K、 p-Akt相对表达水平显著升高(P<0.05);与模型组相比,维酶素组及中药组大鼠胃组织的p-PI3K、 p-Akt相对表达水平显著降低(P<0.05)。本研究发现加味七方胃痛颗粒能有效改善PLGC模型大鼠一般情况及胃黏膜病理状态,降低大鼠血清中的炎症因子IL-6和TNF-α的水平,以及胃组织中“炎癌转化”相关蛋白p53和Ki-67的表达水平,阻断大鼠胃“炎癌转化”的进程,其具体机制可能与调控PI3K-Akt信号通路有关。
Abstract:This study aims to investigate the molecular mechanism of Modified Qifang Weitong Granules in preventing the "inflammation-cancer transformation" of precancerous lesions gastric cancer(PLGC), and to clarify its regulatory effect on precancerous lesions gastric cancer. A PLGC rat(Rattus norregicus) model were constructed using composite modeling method and the rats were randomly divided into control group, model group, Vitacoenzyme group, and Chinese medicine group. HE staining was used to evaluate the pathological changes of gastric mucosa, and immunohistochemistry technique was performed to detect the expression of p53 and Ki-67 proteins. Differentially expressed genes(DEGs) were screened through transcriptome sequencing, and functional annotation and KEGG enrichment analysis were conducted for the DEGs. The enriched key signaling pathways were also verified. The results showed that compared with the blank group, the rats in the model group had significantly reduced body weight and 3-hour food intake(P<0.01), obvious atrophy of gastric mucosal glands, and increased expression of Ki-67 and p53 proteins(P<0.01). Compared with the model group, the Chinese medicine group and the Vitacoenzyme group had significantly increased body weight and 3-hour food intake(P<0.01), improved atrophy of gastric mucosal glands, and reduced inflammatory cell infiltration, and the expression of Ki-67 and p53 proteins was decreased(P<0.01). Transcriptome sequencing revealed 1 197 DEGs between the blank group and the model group, and 1 298 DEGs between the Chinese medicine group and the model group. Visualization of multigene enrichment for both groups of DEGs revealed significant enrichment of PI3K-Akt signaling pathway.Western blot results showed that compared with the gastric tissue of normal rats, the relative expression levels of p-PI3K and p-Akt in the gastric tissue of the model group were significantly increased(P<0.05). Compared with the model group, the relative expression levels of p-PI3K and p-Akt in the gastric tissues of rats in the Vitacoenzyme group and the Chinese medicine group were significantly reduced(P<0.05). In conclusion, the present study reveals that Modified Qifang Weitong Granules can effectively improve the general condition and pathological state of gastric mucosa in PLGC model rats, reduce the levels of inflammatory factors IL-6 and TNF-α in serum and the expression of "inflammation-cancer transformation" related proteins p53 and Ki-67 in gastric tissues, and block the process of "inflammation-cancer transformation". The mechanism may be rela-ted to the regulation of PI3K-Akt signaling pathway.
