重组人脑利钠肽(新活素,rh BNP)已经广泛地应用于治疗慢性心力衰竭急性失代偿性重症(ADHF),给药方式对于治疗ADHF患者临床疗效的影响尚缺乏报道。本研究试图通过对比持续给药和间歇给药两种方式阐明给药方式对其疗效的影响。本研究选择入我院的ADHF患者50例,随机分为对照组和实验组各25人,对照组为持续给药组,实验组为间歇给药组。给药后观察两组患者的心率、每搏输出量、心脏指数、心功能指标、平均动脉压、尿量及血浆脑钠肽水平的变化。研究结果表明:治疗后,两组患者的上述观察指标均得到改善(p<0.05)。持续给药组的疗效强于间歇给药组,但两组无统计学差异(p>0.05)。实验组在血流动力学等参数改善不优于对照组。因此,我们初步认为rh BNP治疗慢性心力衰竭急性失代偿重症患者的疗效显著,但与持续给药方式相比,间歇给药方式无明显优势。因此,我们建议重组人脑利钠肽(rh BNP)在临床治疗应用上应该采取持续给药方式对ADHF患者进行治疗。
Abstract：Recombinant human brain natriuretic peptide(natriuretic peptide,rh BNP) has been widely used in the treatment of ADHF,but the influence of administration methods on the clinical efficacy of ADHF patients still lacks reports.This study attempted to clarify the effect of administration methods on the efficacy of ADHF by comparing continuous and intermittent administration.A total of 50 patients with ADHF in our hospital were selected and randomly divided into experimental group and control group with 25 patients in each group.The control group was continuous administration group,and the experimental group was treated with intermittent administration.The changes of heart rate,stroke volume,cardiac index,cardiac function index,mean arterial pressure,urine volume and plasma brain natriuretic peptide levels in the two groups were observed afte r administration.The results showed that the above indicators in the two groups all were improved after treatment(p <0.05).In addition,the efficacy of continuous administration group was better than that of intermittent administration group,but there was no significant difference between the two groups(p>0.05).Moreover,the improvement of hemodynamic parameters in the experimental group was not better than that in the control group.Therefore,we preliminarily considered that the effect of rh BNP was significant on the treatment of ADHF.However,compared with continuous administration,intermittent administration method had no obvious advantage.In conclusion,we suggested that rh BNP should be used in continuous administration in the treatment of ADHF patients in clinical application.
