为了分析经皮冠动脉介入治疗(percutaneous coronary intervention, PCI)对不同年龄组急性冠脉综合征(acute coronary syndrome, ACS)患者血清重组可溶性CD40配体(recombinant soluble CD40 ligand, s CD40L)和金属蛋白酶-9 (matrix metalloproteinase-9, MMP-9)水平的影响。本研究随机选取40例健康志愿者、就诊的40例急性心肌梗死(acute myocardial infarction, AMI)和40例不稳定心绞痛(unstable angina pectoris, UAP),按年龄≤60岁为一组,>60岁为一组,每组20例。分别在PCI治疗前1 d、治疗后1 d和7 d测定并统计各组间血清s CD40L和MMP-9水平变化。ACS患者血清s CD40L和MMP-9水平均明显高于对照组(p<0.05);>60岁的ACS患者血清s CD40L水平略高于≤60岁患者,但是无显著性差异(p>0.05);PCI治疗1 d后可增加ACS患者的血清s CD40L和MMP-9水平(p<0.01),7 d后则逐渐下降。ACS患者的血清s CD40L和MMP-9水平变化与年龄无明显关系;PCI治疗可能加重了冠状动脉内早期炎症,致使血清s CD40L和MMP-9水平在短期内增加,后期则逐渐恢复正常。
Abstract：To analyze the effects of percutaneous coronary intervention(PCI) on serum levels of recombinant soluble CD40 ligand(s CD40 L) and metalloproteinase-9(MMP-9) in patients with acute coronary syndrome(ACS)of different ages. 40 healthy volunteers, 40 patients with acute myocardial infarction(AMI) and 40 patients with unstable angina pectoris(UAP) were randomly selected. According to age(<60 years old) as a group and(>60 years old) as a group. Serum levels of s CD40 L and MMP-9 were measured before 1 day and after PCI 1 day, 7 days respectively. The serum levels of s CD40 L and MMP-9 in ACS patients were significantly higher than those in control group(p<0.05); The serum levels of s CD40 L in ACS patients over 60 years old were slightly higher than those in ACS patients under 60 years old, but there was no significant difference(p>0.05); The serum levels of s CD40 L and MMP-9 in ACS patients increased after 1 day of PCI treatment(p<0.01), but decreased gradually after 7 days.The changes of serum s CD40 L and MMP-9 levels in ACS patients have no significant relationship with age; PCI treatment may aggravate the early inflammation of coronary artery, resulting in the increase of serum s CD40 L and MMP-9 levels in a short period of time, and gradually return to normal in the later period.
