探讨ASCUS细胞中p16的表达与子宫颈CIN患者的相关性。选取2014年2月至2015年10月本院经宫颈液基细胞学检查诊断为非典型鳞状细胞患者137例,所有患者均经阴道镜取活组织标本行组织病理学确诊,分析不同病检结果的非典型鳞状细胞患者的p16 mRNA和p16蛋白表达水平以及HR-HPV感染率的差异,并分析非典型鳞状细胞患者的p16 mRNA及其蛋白表达与HR-HPV感染率的相关性。研究显示,137例非典型鳞状细胞患者病理结果为阴性38例(27.74%),CINⅠ35例(25.55%),CINⅡ28例(20.44%),CINⅢ22例(16.06%),宫颈鳞癌14例(10.22%)。病检阴性、CINⅠ、CINⅡ、CINⅢ和宫颈鳞癌患者的p16 mRNA阳性表达率(10.53%,45.71%,57.14%,63.64%,71.43%)、p16蛋白阳性表达率(0%,40.0%,78.57%,90.91%,100%)、HR-HPV阳性率(0%,8.57%,35.71%,54.55%,57.14%)均随着宫颈病变程度的加深逐渐上升,且差异具有统计学意义(p<0.05);HR-HPV阳性的非典型鳞状细胞患者p16 mRNA表达阳性率为84.85%,p16蛋白表达阳性率为75.76%,非典型鳞状细胞患者的p16 mRNA、p16蛋白表达阳性率与HR-HPV阳性率呈正相关(rs=0.325,p<0.001和rs=0.303,p<0.001)。p16 mRNA及蛋白表达阳性率随着宫颈病变程度的加深而逐渐上升,可能是由于HR-HPV感染引起p16表达增加,p16 mRNA和蛋白表达阳性率与HR-HPV阳性率呈正相关性,进一步提示了HPV16感染以及其他类型的高危HPV分型对于宫颈病变发生发展的促进作用。
Abstract：To investigate the correlation between the expression of p16 in ASCUS cells and cervical CIN patients,we select 137 cases of atypical squamous cell diagnosed as cervical dysplasia by cervical cytology from February2014 to October 2015 in our hospital. All cases were confirmed by histopathology with colposcopy biopsy. The expression levels of p16 mRNA and p16 protein as well as the difference of HR-HPV infection rate in atypical squamous cell patients with different disease results were analyzed. The correlation between the expression of p16 mRNA and protein and the infection rate of HR-HPV in atypical squamous cell was also analyzed. The results showed that the pathological results of 137 cases of atypical squamous cell were negative in 38 cases(27.74%), CINⅠ35 cases(25.55%), CINⅡ 28 cases(20.44%), CINⅢ 22 cases(16.06%), and cervical squamous cell carcinoma(10.22%). The expression rates of disease negative, CINⅠ, CINⅡ, CINⅢ and p16 mRNA positive expression in patients with cervical squamous cell carcinoma(10.53%, 45.71%, 57.14%, 63.64%, 71.43%), p16 protein positive rates(0%, 40.0%, 78.57%, 90.91%, 100%), and the positive rates of HR-HPV(0%, 8.57%, 35.71%, 54.55%, 57.14%)all gradually increased with the degree of cervical lesions deepening, and the difference was statistically significant(p<0.05). The positive rate of p16 mRNA and the positive rate of p16 protein in HR-HPV positive atypical squamous cell was 84.85% and 75.76%, respectively. The positive rates of p16 mRNA and p16 protein in atypical squamous cell were positively correlated withthe HR-HPV positive rate(rs=0.325, p<0.001 and rs=0.303, p<0.001). The positive rates of p16 mRNA and protein gradually increased with the progression of cervical lesions, which may be due to the increase of p16 expression caused by HR-HPV infection. The positive rate of p16 mRNA and protein was positively correlated with the positive rate of HR-HPV, further suggesting HPV16 infection and other types of high-risk HPV typing could promote the occurrence and development of cervical lesions.
